Client Risk Acknowledgement & Liability Waiver

Information Notice

1. Activities Covered

Activities may include obedience, behavior work, agility, tracking, search, swimming, environmental exposure, personal protection, decoy/helper work, transport, assessment and handler education.

2. Acknowledged Risks

Risks include bites, scratches, falls, collisions, escape, drowning, heat illness, property damage, disease exposure, equipment failure and unpredictable animal behavior.

3. Owner Representations

The owner confirms lawful authority over the dog and agrees to disclose health, aggression, bite history, triggers and restrictions.

4. Assumption of Risk

Participation is voluntary. The client accepts inherent risks that remain despite reasonable professional precautions.

5. Release and Limitation

Any signed release will apply only to the extent permitted by Philippine law and will not exclude liability that cannot lawfully be excluded.

6. Indemnity

A signed agreement may require the client to cover third-party claims caused by the client, guest or dog, including undisclosed risks or failure to follow instructions.

7. Emergency Authority

The signed form may authorize reasonable emergency action and veterinary care when the owner cannot be contacted.

8. Separate Signature

Viewing this page does not create a waiver. A waiver becomes effective only when properly completed, understood and signed.
